If the participant is under 18, a parent or guardian must complete this form. Da Center collects only what is needed to keep young people safe, never publishes personal information, and requires guardian consent and appropriate safeguarding for all youth participation.

Your answers go to a private Da Center staff area. They are never published, and we never ask for payment or card details on this site.
